Patient Bill of Rights


You have the right to:

make a compliant, to be informed of Advocacy, Inc., and receive their phone numbers. The numbers are listed on your written copy of rights.

You have all the rights of a citizen of the State and the United States of America.

You have the right to:

be presumed mentally competent unless a court has ruled otherwise.
a clean and humane environment.
appropriate treatment in the least restrictive setting available.
be free from mistreatment, abuse, neglected, and exploitation.
be told in advance of all estimated charges being made, the cost of services   
provided by the hospital, sources of the program's reimbursement, and 
any limitations on length of services known to the hospital.
fair compensation for labor performed for the hospital in accordance 
with the Fair Labor Standard Act.
be informed of any hospital rules and regulations concerning your conduct and 
course of treatment

PERSONAL RIGHTS

Only your physician can restrict these rights if deemed necessary to protect 
yourself or others.
You have the right to:

use the phone, have visitors, among other usual daily activities.
keep and use your personal possessions
physical exercise and for going outside at least daily.
religious freedom.
opportunities for socialization with persons of either sex.
ask to be moved to another room if another person is disturbing you.
receive treatment of any physical problems which affect your treatment.

You have the right not to be unnecessarily searched unless your physician believes
      there is a potential danger and orders a search.

CONFIDENTIALITY

You have the right to:

review the information contained in your medical record.
have your records kept private and to be told about the conditions under which     
information about you can be disclosed.
be informed of the current and future use of products of special observation and 
audiovisual techniques.

CONSENT

You have the right to:

be transported to, from, and between private psychiatric hospitals in an appropriate 
way.
and individualized treatment plan in which you take part.
be informed of your care.
receive information regarding your medications and a list of what they are.
refuse medication
meet with the staff responsible for your care.
request a second opinion.

You have the right not to be given medication you don't need.

VOLUNTARY PATIENTS

As a voluntary patient you have special rights which are listed on page 3 of your handout.

You have the right to request discharge and have that request addressed within four 
hours.
You have the right not to have court ordered services while you are a voluntary patient unless your physician determined you meet the criteria for court ordered services.

The law is written to ensure that people who do not need treatment are not committed.
